Use as directed by a doctor. Preparation and Use: Your baby's health depends on carefully following these directions. Failure to follow these directions could result in severe harm. Ask your baby's doctor if you need to use cooled, boiled water for mixing and need to boil (sterilize) bottles, nipples and rings before use. 1. Shake very well before each use. Remove protective seal around bottle cap; twist off and clean cap. 2. Invert cap and place on foil seal. Press down to pierce foil, then turn cap a half turn. 3. Pour equal amounts of formula and water into bottle or cup. 4. Attach nipple or lid; swirl gently to mix. Once feeding begins, use within 1 hour or discard. Prepare and feed all formula from this container within 4 days after opening. Storage: Once opened, store quart bottle in refrigerator. Once prepared, store bottles in refrigerator and feed to baby within 48 hours. Store unopened containers at room temperature; avoid extreme temperatures. Do not reuse container. Contains milk and soy ingredients. Do not use this container to warm formula. Never use a microwave to warm formula. Serious burns can result.
